No. If you are taking your husband or wife’s surname, your marriage certificate is your proof and that is all most organisations will ask for. What you may still want is the Name Change Kit, because the certificate proves the change but does not tell you who to send it to.
Usually not. Your decree absolute together with your birth or marriage certificate is normally enough. If an organisation refuses those, or you want a name that is neither your maiden name nor your married name, then you need an Adult Deed Poll.

From 16 a young person normally applies in their own name, so that is the Adult Deed Poll. The Child Deed Poll is for 15 and under, where a parent or carer applies for them.
Most adults find around ten is right, because every organisation wants to see proof and some keep a copy. Families changing a child’s name usually need fewer, around five. You choose the number when you apply.
