Your witness must be aged 18 or over, live in the UK, and be independent of your family.

  • Support by email and phone

A real person to answer questions before, during and after your application.

How it works

Four steps to your child’s new name

  1. Apply online

About five minutes. Your child’s current and new name, your details as the parent or carer, and how many certified copies you want.

  2. We prepare it

The wording is checked by hand for spelling and legal accuracy, then printed and dispatched.

  3. Everyone signs

Each person with parental responsibility signs in front of an independent witness aged 18 or over.

  4. Update their records

Send a certified copy to the school, the GP surgery, HM Passport Office and anyone else who holds your child’s details.

Digi-Poll™ Archive

Your child’s details, on file for the next ten years

A school, a GP surgery, a passport renewal, a sports club, a new dentist. Each one asks for proof of your child’s name at a different time, and each may want its own certified copy. The Digi-Poll™ Archive keeps their change of name details on file so you are ready every time.

  • Held safely on file

Your child’s change of name details are stored securely, so nothing is lost if your own paperwork is.

  • Reorder in a minute

Need another certified copy before the next school year? Order it from your archive instead of filling in a fresh application.

  • Over 75% cheaper

Certified copies ordered from your archive cost a fraction of what starting a fresh application would cost.

Add the Digi-Poll™ Archive as an optional extra when you apply.

Questions

Frequently asked questions

Who can apply for a child deed poll?

A parent or carer with parental responsibility for a child aged 15 or under. You complete the application and sign the deed poll on your child’s behalf.

Does everyone with parental responsibility have to agree?

Yes. Every person who holds parental responsibility for the child must consent to the change of name and sign the deed poll. If someone will not consent, the name cannot be changed without an order from the court.

If you are not certain who holds parental responsibility for your child, take legal advice before you apply.

My child is 16 or 17. Which deed poll do we need?

From the age of 16 a young person can normally apply in their own name using our Adult Deed Poll.

How long does it take?

Applications received before 3pm, Monday to Friday, are processed the same day. Documents are dispatched within 2 working days by First Class Post. Once the deed poll is signed and witnessed, the new name is in effect immediately.

How many certified copies do I need?

Most families need fewer copies than an adult does. Think of the school, the GP surgery, the passport application, and any club or activity that holds your child’s records. Around five is a common choice, and you select the quantity when you apply.

Who can witness a child deed poll?

A person aged 18 or over who lives in the UK and is independent of your family. A witness cannot be a relative of the child, or anyone who lives at the same address.

Will the school and HM Passport Office accept it?

Yes. Your child’s deed poll is guaranteed to be accepted as evidence of their new name by schools, HM Passport Office, the NHS, the DVLA and every other relevant organisation. If it is refused, we refund you in full.
