Yes. Every person who holds parental responsibility for the child must consent to the change of name and sign the deed poll. If someone will not consent, the name cannot be changed without an order from the court.

From the age of 16 a young person can normally apply in their own name using our Adult Deed Poll.

Most families need fewer copies than an adult does. Think of the school, the GP surgery, the passport application, and any club or activity that holds your child’s records. Around five is a common choice, and you select the quantity when you apply.
A person aged 18 or over who lives in the UK and is independent of your family. A witness cannot be a relative of the child, or anyone who lives at the same address.
Yes. Your child’s deed poll is guaranteed to be accepted as evidence of their new name by schools, HM Passport Office, the NHS, the DVLA and every other relevant organisation. If it is refused, we refund you in full.
